  • Completed Application, pages 1 and 2.
  • Completed Authorization to Release Information on Occupancy Form.
  • $39.95 application fee in the form of cash, money order or credit card.
  • Two most recent paystubs (per employer).
  • Any forms verifying other income (award letters, etc).
  • Copy of photo id (i.e. Driver’s license) and social security card.
  • Copy of forms showing favorable status with Banking Institution.
  • Copy of W-2’s for current tax year and prior tax year, if required.
